Signos de peligro en el paciente con malaria

Danger signs are clinical indicators of severity and are useful to predict complications or death. In the malaria patient, clinical or parasitological signs can be easily be recognized during the acute phase of the illness that indicate serious complications. Danger signs include neurological change, abnormal breathing pattern, persistent vomiting and diarrhea, jaundice, bleeding, dark urine, delayed capillary refill, intense pallor, hyperpyrexia, hyperparasitemia and schizontemia. Timely recognition of these signs can lead to a decrease in cases with complications and deaths.

La respuesta de shock térmico y el dimorfismo en Leishmania

Protozoa of the Leishmania genus are intracellular parasites having two different stages, promastigote and amastigote, which are present during their life cycle in the invertebrate host and the mammal host respectively. During its life cycle, the parasite suffers dramatic temperature changes and the transformation success between one and the other form is crucial for its survival and reproduction. The response of the organisms to temperature or heat shock response has been well studied in Leishmania, promastigotes having been obtained at 26°C and amastigotes at 35°C respectively, and their reconversion when the temperature is increased or lowered has been demonstrated. These changes in parasite morphology are accomplished by biochemical and molecular variations with differences in enzyme activity and protein synthesis. These processses increase the production of heat shock proteins, specifically HSP70 and HSP83, which continue to be expressed as pari of the Leishrnania amastiyotes as a way of adaptation to vertebrate host environment. At a molecular level, it has been demonstrated that almost al1 heat shock proteins structural genes are arranged in tandem repetition, being transcribed in polycystronic RNA and finally processed by poly-adenylation and trans-splicing. Nevertheless, it has not been possible to identify any transcription promoter in these parasites. Furthermore, studies of transcriptional regulation have demonstrated that the regulation of heat shock proteinsr gene éxpression in Leishmania occurs by post-transcriptional processes. This demonstration has marked a fundamental difference between Leishmania and the other non-protozoal eucariotes.

Tratamiento de las infecciones maláricas producidas por el Plasmodium Falciparum y el Plasmodium Vivax

The treatment of P. falciparum resistant to chloroquine will depend upon the clinical features of acute falciparum infection, the susceptibility of the parasites to antimalarial drugs and the level of immunity of the host. Serial parasite counts/m.c.l of blood should be made in order to select the treatment and evaluate the efect on the parasitoemia. Patients with parasite counts below 100.000/m.c.l of blood should be treated with quinine sulfate for three days in association with sulfadoxine-pyrimethamine. If the patient has a high parasite count ( 100.000/m.c.l of blood) or a complicated infection, quinine dihydrochloride should be administered as a slow intravenous infusion, at a dose of 7-10 mg/kg every 8 hours. A variety of treatment regimens combinign different antimalarials are recommended, if there is a subsequent recurdescence of the infection to precious courses. Special care is needed when treating acute infections with multidrug-resistant porosites. in orden to evaluate the reason why an infection fails to respond to standard antimalarial therapy.

The clinical and histopathologic picture of four patients with diffuse leishmaniasis (DL) from four different areas of Colombia are presented. The initial diagnosis was made Ly histopathology in all of them. A 10 year old girl and an 80 year old male had disseminated lesions; the other two cases were two men of 33 and 62 years old who presented with nodular lesions treated succesfully with local heat and Glutantime (R). The four cases had a negative Montenegro test. We confirm that DL may be cured when treated early before it becomes a disseminated disease. Local heat was a powerful tool in treating one of the two cases with a unique nodular lesion. L. m. amazonensis was identified in this case by culture and isoenzyme analysis and L. mexicana was identified in the 80 year old patient by the same procedures. Hamsters inoculated with suspensions of the ground biopsies developed huge mouth and footpad histiocytomes. We illustrate the clinical aspects of the disease and discuss the histopathology and the electronmicroscopic leishmania-macrophage relationships. An overview of the characteristics of this entity and a review of the literature are presented.

Disseminated strongyloidiasis (DS) is an opportunistic disease, fatal in 50% of the cases, and due to internal autoinfection with larvae of S. stercoralis. We reviewed DS in more than 14.000 autopsy protocols in Bogotá, from 1954 to 1986, in order to know the basic diseases which promoted the larvae dissemination, and also, to investigate if DS had increased recently. We found only twelve patients, 1-62 years old, dying of DS. Patients presented with smelly and bloody diarrhea, vomit, abdominal tendemess, anorexia, cough, and expectoration sometimes homoptoic. All patients had advanced malnutrition and only one han been treated with steroids and cyclophosphamide because of progressive glomerulonephritis. One patient was treated with steroids for "ulcerative colitis", which aftenvards was proved to be due to DS. Only once, a clinical diagnosis of DS was done. Strongyloides larvae were found in the stools of 3 patients, but the strongyloidiasis was not considered as disseminated. Clinical diagnosis included: malnutrition, anemic syndrome, gastric carcinoma, intestinal and pulmonary tuberculosis, intestinal parasitism and meningeal syndrome. One patient had bacterial tricuspideal endocarditis, and two, meningoencephalitis, together with the intestinal syndrome. The meningeal infection was of acute bacteria1 type, with no granulomas or larvae. These were found, with granulomatous inflammation in the small and large bowel, lung, liver and mesenteric lymph nodes. In four patients trombosis of great venous vessels was found, in the inferior vena cava, femoral, mesenteric and superior longitudinal sinus Fatal DS is not frequent and has not increased recently among the population studied. It should be suspected in patients with diarrheic syndrome and malnutrition. Slight blood eosinophilia (5 and 7%) was present only in two patients.

Enfermedad de Jorge Lobo o Blastomicosis queloidiana. Nuevos aspectos de la entidad en Colombia. Revisión

Lobo's disease is a subcutaneous mycosis which was first described by Jorge Lobo in 1931 in a patient fron the Brazilian Amazon region. The first colombian case was reported in Cali (Universidad del Valle) in 1958. Lobo's disease in humans is unique to Latin-America. Over 260 cases of this disease have been reported, 135 from Brazil, 2 1 % of which have occurred among the Cayabi indian tribe. Until 1982, only nine colombian cases had been reported. Two of these were published twice, signifying that the acutal number of cases are seven. These were either mestizo or negro patients who attended teaching on local hospitals. In this paper we report ten cases of Lobomycosis is Amer Indians patients (Curripaco, Cubeo, Cuiba and Guahibo tribes), from Guainía, Vichada, Casanare and Vaupés regions. Their disgnosis was confirmed by skin biopsies sent to the National Institute of Health in Bogotá by physicians serving a compulsory year of social service. There are no previous reportes of this disease among colombian Amer-Indian communities. We also reviewed eigth patients either referred to us by colleagues or presented at the XIV National Congress of Dermatology, in 1984. Thus, the current number of reported cases of Lobomycosis in Colombia has risen to 2.5, making it the third highest frequency in the world behind Brazil and Surinam. The geographical regions where this disease is prevalent in Colombia are the Orinoco and Amazon rivers basins, where Amer-Indians patients are affected, and the Pacific Coast, where the black patients come from. Lobomycotic lesions are plaques and nodules of chronic evolution, which occur predominantely in male patients. Lobomycosis, most common complication is ulceration, making leishmaniasis the most important differential diagnosis. One Amer-Indian patient developed a squamous cell carcinoma over a Lobomycotic lesion with two-years of evolution. This patient was completely cured after surgical excision of the lesion. A general review of Lobomycosis is made and macro and microscopic features are illustrated.

Adhesion molecules (AM) are involved in antigenic recognition and cell migration. These molecules determine the contact among several groups of cells and the relationship between some cells and the extracellular matrix. In addition, they act as direct activation signals upon various immunocompetent cells. According to their structure and functional characteristics they have been grouped in: lnmunoglobulin superfamily, integrins (very late antigens, leukointegrins and cytoadhesins) and selectins (E-selectins, P-selectins y L-selectins). Their mechanisms of action have Seen largel y studied in regard to the leukocyte migration process, which include: selectin-mediated margination; endothelial adhesion and chemotaxis involving integrins and in a less extent by the immunoglobulins superfamily members. The possibility exists that manipulation of important steps of these processes could be used in the future as therapeutic tools and basic research.

Since the discovery of trophic factors, their role in the development, maintenance and regeneration of the nervous system has been postulated and in some cases, proven. The knowledge and manipulation of these substances has been used: i) to formulate new theories about the etiology of various neurodegenerative disorders and ii) to develop new therapeutic alternatives in the treatment of these diseases. In this review, we summarize the main effects of several trophic factors on the developing dopaminergic neurons of the substantia nigra, whose degeneration causes, in the adult, most of the symptoms seen in Parkinson's disease.

The scope of ethics in health research transcends its legal framework and the regulations established in Resolution 8430 of 1993. These norms represent a fundamental tool to determine the minimum protection standards for research subjects, and, therefore, they should be known, applied properly, and reflect upon by all researchers in the field.
Here I present and discuss from an analytical point of view the regulations that guide research in health. In this framework, health is understood as a multidimensional process, and research in health as a multidisciplinary exercise involving basic, clinical and public health research, collective health, and other related sciences.
The main analytical categories are related to the principles and actors involved in research (regulatory authorities, ethical committees, and special or vulnerable subjects and populations), and to professional ethics codes, in addition to informed consents and data management.
Despite the contribution of this legislation to the qualification of health research, my conclusion is that the national legislation in ethics for health research requires updating regarding technological and scientific developments, as well as specifications from the multiple types of health studies.

Los coronavirus son una familia de virus que se caracterizan por producir afectaciones respiratorias y gastrointestinales en animales y en seres humanos. El actual SARS-CoV-2, agente infeccioso de la COVID-19, pertenece a un subgrupo denominado betacoronavirus del que hacen parte el SARS-CoV y MERS-CoV, virus responsables de epidemias en el 2002 y el 2012, respectivamente.
Estos virus también pueden infectar el sistema nervioso debido a su afinidad con la enzima convertidora de angiotensina humana 2 (ACE2), la cual se expresa en neuronas y células gliales. Se ha demostrado que las infecciones con SARS-CoV y MERS-CoV, y ahora también con el SARS-CoV-2, ocasionan condiciones neurológicas como la enfermedad cerebrovascular aguda, la conciencia alterada y las lesiones musculares, así como mareos, hipogeusia, hiposmia, hipoxia, neuralgia y encefalopatía hipóxica. Por ello debe prestarse mucha atención a las manifestaciones neurológicas de los pacientes de COVID-19.

Retos y oportunidades en el estudio de vesículas extracelulares: contexto institucional a nivel mundial y situación actual en Colombia

En la última década se ha incrementado el número de estudios y publicaciones sobre las vesículas extracelulares y los exosomas. En Colombia, ha habido interés y avances en su estudio, lo que se evidencia en el aumento de publicaciones y proyectos de investigación. Sin embargo, este es un campo de investigación aún en desarrollo, con desafíos analíticos y limitaciones técnicas, por lo cual, en el planteamiento de los proyectos de investigación y desarrollo, es necesario considerar cuál es el estado del campo científico a nivel mundial en cuanto a la nomenclatura y la clasificación de las vesículas extracelulares, las técnicas, recursos, requisitos y especificaciones de calidad y las instituciones que regulan el campo. La respuesta a esta pregunta permitirá desarrollar estudios que cumplan con los estándares internacionales, y las exigencias y recomendaciones institucionales. Sin embargo, la información científica disponible se encuentra dispersa y no todos los aspectos son tratados a cabalidad.
En este actualización se condensa la información disponible y se presentan los términos oficiales para denominar las vesículas extracelulares y la nomenclatura aceptada actualmente, así como la evolución del campo, la homogenización de los parámetros experimentales, el establecimiento de autoridades científicas, instituciones y recursos, y las recomendaciones que se han generado a nivel mundial para el desarrollo de investigaciones en vesículas extracelulares, incluidos su aislamiento, caracterización y estudio funcional. Por último, se analiza el contexto nacional de una forma crítica, teniendo en cuenta las fortalezas institucionales, los errores usualmente cometidos, y las técnicas y tecnologías analíticas disponibles.

El monitoreo electroencefalográfico no invasivo continuo es una técnica indispensable en los pacientes neurológicos críticos, ya que muestra de forma directa e indirecta su actividad cerebral y permite relacionar los hallazgos con su estado clínico. Es muy sensible, aunque su especificidad es menor, por lo que puede demostrar la alteración del estado de conciencia sin aclarar su etiología.
El uso del registro electroencefalográfico continuo en pacientes con alteraciones del estado de conciencia, convulsiones, o estado epiléptico convulsivo y no convulsivo, se ha incrementado en los últimos años porque permite obtener información en tiempo real de la función cerebral y de los cambios en el tiempo; además, facilita la detección de crisis epilépticas subclínicas y electrográficas, estas últimas de gran importancia, ya que no presentan correlación clínica.
Los hallazgos del monitoreo permiten modificar el tratamiento farmacológico y anticonvulsivo, y constituyen una gran ventaja para el médico tratante en la toma de decisiones oportunas que redunden en la mejoría del pronóstico del paciente.
